Forms

Parents/students may need forms provided for the entire school district. Those forms may be found on the CCSD Forms page.

 

 PARKING 

Parking Permit Fee: $100 (Full Year) 2nd Semester $50 (January- May) Credit Card payments, online only.

You MUST provide the following items, including COPIES of all required documents.

•        Completed  2025 - 2026 PARKING PERMIT APPLICATION  (parent signature is required on the application unless student is 18 years old)
•        Copy of Driver's License (no permits)
•        Proof of valid car insurance, PLUS proof that student is covered on the policy, as required by law.
•        Proof of valid car registration (photos of license plates will not be accepted)
•        Proof of payment

 ** If you have any unpaid fees or fines including parking tickets, textbooks, media center, etc you must pay them before you can purchase a parking pass for the new school year. **

Certificate of School Enrollment Form - needed for DDS (drivers license/permit)
